Intoduction
As a professional pregnancy and childbirth advocate, I draw on my knowledge and experience to provide emotional support, physical comfort and informational support. I am a non-clinical care provider and am not licensed as a medical care provider. I am available to provide communication with the staff as needed, to make sure that you have the information you need to make informed decisions as they arise in pregnancy and labor.
I am independent and self-employed. For references and information on my personal experiences, please do ask. I will meet with you and your partner to discuss your priorities and concerns and to plan how we might best work together. At that time, we can discuss fees. This initial meeting does not obligate you to use my services and is completely free.
Upon signing the contract please initial the services you wish to contract for and sign in the appropriate areas. Fees are due as designated at signing of contract and can either be sent using the enclosed SASE or in person at our first meeting.
Childbirth Education
I will provide a maximum of 8 hours of childbirth education at your home, provide information on pregnancy, birth, breastfeeding and baby care, answer questions and help with your birthing preferences. (*NOTE Education is included in Prenatal, Labor and Birth Support.)

Prenatal, Labor and Birth Support
I am available for phone calls and scheduled prenatal meetings – as many as required. I am available for doctor appointments, testing, and non-stress tests as needed. I will provide documents and information regarding pregnancy; birth, breastfeeding and baby care as requested. My services include physical, emotional and informational support. I will do light housekeeping, errands and support in-home for high risk and bed rest clients.
I require 2 prenatal meetings to gather information and help with your birth preferences; more are offered at your request. I am on call 24/7 from your 38th week to the birth of your baby. I will provide early labor support as requested, including in your home. I will be a continuous presence during labor, as allowed, from the time of arrival until approximately 2 hours after birth. I will utilize non-medical support techniques for labor and birth and will strive to maintain a calm and peaceful birth environment. I will provide one post-natal visit to discuss the birth and deliver a birth story.
I am available to you by phone or in person from signed date. I am on call from your 38th week till birth, however if labor begins earlier or complications arise I will attempt to make myself available for support. (*NOTE If you choose doula support after CBE series, all monies paid will be credited to birth doula support.)

Postpartum Support
I am available by phone and in person from the birth of your baby up to 6 weeks postpartum for emotional, physical support, baby care and feeding support. I will help with information; comfort measures, baby care issues and am available for light housekeeping, errands and support in-home as needed.

LIMITATIONS As a doula, I do not perform clinical tasks such as blood pressure readings, fetal heart checks, vaginal exams and others. I am not responsible for the clinical/medical well-being or you or your baby. I am there to provide physical comfort, emotional support, information and advocacy. I will not make decisions for you or your partner or contradict medical care advice from your provider.
I will help you get information so you can make informed decisions and then support you in those decisions. You and your partner are in complete control, I work for you, not your provider, hospital or staff. I will not make choices for you or interfere with your choices. It is your responsibility to be informed, make decisions and inform your health care provider and myself of those decisions. I will however offer information about options available and evidence based information for you to research. If you do not want information on your choices and options you must inform me of this fact. I will offer advocacy support for the choices you have made me aware of, but will not question nor contradict staff, medical advice or your provider.
Client Responsibility
You and your partner are responsible for making pregnancy, birth and postpartum decisions. You must clearly inform your provider, staff and myself of your wishes, wants and needs. If your wishes, wants or needs differ from that of your provider or partner it is imperative that I know these facts. It is of utmost importance that I be kept informed of your health, the health of the baby and any complications that arise so I can support you as needed.
You and your partner are responsible for being informed and obtaining need to know information about your pregnancy and health from your health care provider. If your do not want to be informed of options or information you must tell me as my job is to keep you fully informed. Some of the options and information given may be contradictory of the information or advice given to you by your provider. Remember that this is not the doula contradicting the medical advice of your provider; it is the doula providing options and evidence based information. It is your responsibility to be educated about your choices and options and to make the decisions that are best for you.
For Birth Support or Full Service Support a 50% non-refundable retainer is required at signing and the remainder by 2 weeks postpartum.
All other fees for service are due upon signing of this contract and are non-refundable.
